Provider Rate Form

Please report your rates in
half day and full day
formats for:

infants (0-3 years)
preschool age (3-6 years)
school age (6 years and up)

Child Care Resources cannot calculate amounts for half day and full day based upon weekly or monthly rates nor can we assume the amounts you are charging if boxes are left blank.

We will not be able to enter rates correctly in the system for Best Beginnings Scholarship payment purposes and it can hinder authorizations from being set and pending invoices from being paid.
